We can also take over an existing plan by assuming the agent-of-record position if desired by the employer.
The required information needed to quote on a case can be either emailed or faxed in, or we can set up a meeting to discuss the employer's needs prior to quoting.
Oakland Group Services currently services over 60 employer groups in southeast Michigan totaling over 1,000 employees.
Soon to be available website will offer online quoting for both groups and individuals, online enrollment options, downloadable employer and employee forms, direct links to insurance carriers, and educational information for both employers and employees.
The following information is necessary to prepare a proposal for a typical employee benefit plan:
Group Medical & Dental
Employee census which includes the following:
Name
Social Security number
Age or date of birth of all family members
Gender
Family status (single, two person, family)
Nature of business
Details on current coverage
Current monthly premium invoice
Renewal date of current coverage
- If experience rated or self-funded, two years experience data from current carrier including large claims, headcount, pooling level, and as much detail as possible
Group Life & Disability
All of the above items
